The City of Maple Plain has completed its After Action Report regarding the July 11–12, 2026 water system pressure loss incident and precautionary boil water advisory.
The report documents:
- The events that led to the loss of water pressure.
- The City's emergency response efforts.
- Findings from investigations conducted by the City Administrator and Total Control Systems (TCS).
- Laboratory testing results confirming the absence of bacterial contamination.
- Public communication efforts during the advisory.
- Corrective actions already implemented.
- Recommendations to improve system reliability and emergency preparedness moving forward.
The investigation determined that the incident was caused by a radio communication failure between the Water Tower/Well No. 1 site and the Water Treatment Facility, combined with disabled communication failure alarms. The report also identifies additional contributing factors and outlines steps being taken to reduce the likelihood of a similar incident occurring in the future.
The City appreciates the patience and cooperation of residents, businesses, utility operators, consultants, public safety personnel, and regulatory agencies throughout this event.
